What Is a Chain of Custody Form?

A COC is a record documenting the collection, transfer, receipt, analysis, storage, and disposal of a sample. At every stage, someone is accountable for the sample in their possession.

An incorrect COC can result in vague sample information, our analysts may not know what needs testing. That's why it is important to fill out a COC in its entirety.

Client and Project Information

Fill out company, name, phone, billing address, email, and project number in the upper right-hand corner. Include the sample address, where the sample was taken from.

Sample Information

Label each sample, list room location and material description, note who sampled the material, and who relinquished the sample to Techtron.
